Episode 7 changes everything. For the first time, we get to see the dreams exactly as Helaena experiences them. There are four distinct dreams, and each one is packed with clues about what’s coming.

Not yet—and probably never. This was a dream sequence. In the books, she never rides Dreamfyre into battle. The Storming of the Dragonpit happens after her death.
Almost certainly. In Fire & Blood, Helaena and Aegon have a son named Maelor. The show has heavily hinted at her pregnancy this season. A male heir = political danger for Rhaenyra.
Because it’s not real life—it’s an afterlife vision. Jaehaerys was murdered as a boy (Season 2). Seeing him grown up = Helaena’s wish for what could have been, now playing out in a peaceful "heaven."
It’s a biblical reference (Revelation 6:8 — "Death rode a pale horse") and was used in Game of Thrones Season 5, Episode 8 ("Hardhome") when Arya sees a pale horse after the massacre. It universally means Death is coming.
